Heartfelt Connections is a memory care program designed for individuals with Alzheimer’s disease and other types of dementia. The philosophy focuses on the individual, and it highlights the skills and abilities they can still do over what has been lost.
Our skilled team uses reminiscence tools to help family members capture the unique details of their loved one’s life. This includes their past careers, interests, likes, and dislikes. We use these personal stories to create a care plan. This plan improves physical and emotional health, supports daily activities, and promotes a sense of purpose and fulfillment.
